My vote at this point is that that cylinder isn't firing. Have you pulled the plugs and checked for spark? Have you checked compression in all four cylinders? Have you ever pulled the intake manifold to see the condition of the rats nest of vacuum and air hoses under it?
If you haven't already, I would start with checking the spark and the compression and go from there. But it sounds to me like a bum cylinder.
