I have a feeling it is something in the #3 cylinder. A very small object can make a noise like you described when being slapped around on top of a piston.

It probably would have spit out any foreign object by now too. Was just a guess, I have heard noises like that and it was always either a piece of carbon or some other foreign object like a nut or screw out of a carburetor or something accidentally dropped in the intake.

I was referring to the top of the lobe, not the bottom where you have it set to check the clearance. It can wear and make a bad noise no matter how the clearance is set.
