I'd put my money on bent or worn shift fork, as exavidhas said.
It sounds like you're already passed the point of damage to the engagement dogs (ears) on the gear itself, so be preparred to replace both the shift fork and gear. If the gear is unavailable you may be able to undercut the engagement faces yourself, don't get it to hot while grinding or you'll need to re-heat treat the gear.
