Actually in some states using earplugs is illegal. It's believed you won't be able to hear emergency vehicles. I can't imagine why anyone feels they need them if they have a 3/4 or full face helmet.

As for loud pipes I think it has something to do with p***s envy, mine's 'louder than yours'. All it does is turn lots of people against bikes in general. As for increasing safety I don't believe it, probably gets people peed off enough to want to cut them off on the road.
